We accept Medicare, Medicare Railroad, Tricare, Triwest, IEHP, Cover California, PPO plans including but not limited to Anthem Blue Cross, Blue Shield, United Healthcare, Cigna, HealthNet, and Aetna. We are also contracted with selected local IPAs. Please call our office to verify coverage of your insurance.

Prior to your office visit for a biopsy, we require a Requisition Form or your physician’s order, insurance information including current insurance identification card, photo identification, any insurance authorization from your IPAs, your referring physician’s notes, and, if applicable, any relevant radiology reports be faxed to our office at 951-808-0550.
For patients with participating insurance, applicable co-payments will be collected from you on the day of the procedure. For non-insured patients or those with non-participating insurance, we offer discounted payment arrangements. If you have questions regarding your insurance coverage prior to your visit, please call our office at 951-808-8863.
